Project description

Chronic conditions require an overwhelming therapy. Nurses are a valuable option to provide patient centered guidance.

The aim is to develop nursing consulations for patients with multimorbidity.

Qualitative interviews with patients and providers will be used to explore the necessary content.

After implementation, mixed method research will be used to adapt and test the intervention.
