The transition from prison to the community presents significant risks and challenges, particularly for individuals with substance use problems. However, access to addiction care post-release remains a significant concern for this vulnerable population. Ensuring continuity of care during this transition period is crucial for reducing recidivism and improving health outcomes. From both a recovery and reintegration perspective, this project seeks to explore the barriers and facilitators to effective continuity of care. A mixed-methods approach will be employed, incorporating quantitative data from health care referrals, as well as qualitative insights from interviews with formerly incarcerated individuals, health care providers, and policymakers. The project aims to identify gaps in service provision, explore policy-level interventions that can enhance post-release support systems, and contribute to further developing recovery theory.
